How to Apply for Membership

You may apply for membership in person or you may request a membership packet be mailed to you by calling 615-313-5585 or 800-235-0403 between the hours of 8:00am – 4:00pm (CST), Monday through Friday. If you prefer fill out and submit a Membership Applicaiton.

Individuals who are applying for membership must supply:
  • a completed membership application,
  • a minimum deposit of $5.00 to a regular share savings account,
  • a copy of your driver's license or other proof of identity. We may also ask to see other identifying documents.
  • and, if applying on the basis of employment in a Select Employment Group, a pay stub or other proof of employment in that group may be requested.

Individuals who are applying based on a household affiliation with another member must also supply:
  • all items listed above plus your sponsoring member’s name & phone
